Les compartimos esta versi\u00f3n actualizada en el archivo adjunto. Tambi\u00e9n lo pueden descargar de nuestra p\u00e1gina web<a href=\"https:\/\/www.sosunc.org.ar\/web\/wp-content\/uploads\/2022\/11\/Plan-discapacidad.-Declaracion-Jurada.pdf\"> AC\u00c1<\/a>.<\/p>\n\n\n\n<p>El nuevo formulario corresponde a la Declaraci\u00f3n Jurada para la cobertura de las terapias anuales y la medicaci\u00f3n prescripta por su m\u00e9dico\/a especialista. Es el que tendr\u00e1n que presentar para el a\u00f1o 2023.&nbsp;<\/p>\n\n\n\n<ol start=\"1\" class=\"has-black-color has-text-color wp-block-list\" style=\"color:#000000\"><li>Cada a\u00f1o, hay que renovar el Protocolo M\u00e9dico Anual para solicitar una nueva cobertura durante el per\u00edodo que va del 1 de enero al 31 de diciembre. <\/li><li>Se presenta el Formulario que compartimos junto con los Planes de Trabajo por terapia. Adem\u00e1s, si se trata de una renovaci\u00f3n, se agrega el Informe de Evoluci\u00f3n del m\u00e9dico\/a especialista y un informe de evoluci\u00f3n por cada terapia.<\/li><li>Una vez que el equipo de Auditor\u00eda de SOSUNC aprueba la documentaci\u00f3n presentada, inicia la cobertura.<\/li><\/ol>\n\n\n\n<p>Pueden presentar la documentaci\u00f3n para el a\u00f1o 2023 en forma virtual al correo electr\u00f3nico de la delegaci\u00f3n de su localidad. Se ruega enviar los archivos en formato PDF con una imagen completa y n\u00edtida.
