Por eso, adem\u00e1s de abonar la correspondiente consulta m\u00e9dica, fij\u00f3 el valor extra de otra consulta por la tarea de llenar esta planilla.<\/p>\n\n\n\n<p>El Formulario de Ingreso al Programa Diabetes contiene informaci\u00f3n indispensable para otorgar cobertura a cada tratamiento.
